The medical release form assists Wave Baseball staff in determining the correct care for your player in case of injury or illness when a parent or guardian is not available.  Please fill out the form completely.

This form informs the parents or guardians as to the inherent risk associated with playing sports and gives Wave Baseball staff the right to seek treatment in the cases of injury or illness to a player during a Wave Baseball sanctioned activity.

The purpose of this agreement is to ensure all parties understand expectations of the players and parents in order for everyone to enjoy their Wave Baseball experience.
